Silver Lake Health Verification Information

The following is a list of the physical exam requirements for the BSN completion nursing students at Silver Lake College:

1. TB skin test
2. Rubella immune status and Rubeola immune status blood tests included below OR two doses of MMR (measles, mumps, rubella vaccine)
3. Tetanus booster (required within 10 years of starting date)
4. Hepatitis B Vaccine (optional)

Vaccinations, TB skin tests, and Tetanus boosters are available from the public health nurse’s office (Manitowoc, 920.683.4155; Sheboygan, 920.459.3030) for a small fee. Please call ahead to schedule an appointment.  Use the Student Entrance Health Verification Form to document and verify your vaccination history.
